Applicants must have a documented disability history with the Regional Center.
Applicants must be between 18 and 59 years old.
Applicants should demonstrate a willingness and motivation to participate in the offered services.

Application Process

Receive referral packet from the Regional Center.

Assessment & Orientation

Assessment: Our team will conduct an assessment review with an individual to determine eligibility, areas of need and support.
Assessment Recommendation: Send assessment recommendation to the Regional Center.
Authorization: Receive service authorization from the Regional Center.
Commence Services: Once authorization is received, an individual will be contacted to commence services.

The intake process typically takes 2-4 weeks, depending on the availability of all parties.

What if I am not eligible for the services I am interested in?

Our team will work with you to explore alternative services or resources that may better suit your needs and abilities based on your person-centered plan.

Yes, you can participate in multiple services if it is approved by the Regional Center.
